Immunization Policy
Entrance Requirements
All freshmen must have proof of the meningitis vaccine.
If you were born after 1956 you must provide proof of two measles containing vaccinations, one of which must have been an MMR after 1980, before you can complete the registration process for class.
All new students must provide acceptable proof of appropriate tuberculosis screening within the last year as a registration requirement.
Medical History
You are strongly encouraged to complete the medical history form and the Pharmacy Information Sheet prior to registration.
In addition, you are urged to see your family physician to obtain the required screenings and present acceptable documented proof to the SHC during student orientation. This information should be mailed or faxed to:

If you have any illness or medical condition that requires regular treatment or alteration of your life style, please have your treating physician provide pertinent information and guidelines that will allow the SHC to assist your physician in continuing to provide you with the appropriate care.
